Effect of hand reflexology Massage Method on patients pain and mood following a spinal surgery

Abstract
The purpose of this study was to test the effects of hand feflexology Massage on pain and mood in patients who have just had a spinal surgery as a possible nursing intervention.

The research design was a nonequivalent control group, non-synchronized design. The participants for this study were 40 patients with spinal disease on third day after spin operation. The first 20 patients were assigned to the control group and the second 20 to the experimental group. The study was conducted from February, 1, 2003 to July. 18, 2003. on a neurosurgery unit of E Medical Center (University Hospital) in Seoul, Korea.

Pain intensity, blood pressure, pulse rate and mood were measured as dependent variables. The instrument used for pain intensity and mood was a 10cm visual analogue scale Cronbach alpha for this study was .84.

For the experimental group are and post tests were done immediately before and immediately completion of and reflexology Massage method. Hand reflexology massage method was performed oncs for 10 minutes. After pretest, patients in the control group were instructed to have a quiet time & deep breating & explain & information list given and then post test was done.

The data analysis was done using percentage, frequencies, and chi-test paired t-test with SPSS/PC+program. The number of participants was small so the data were analyzed by Mann-whitney U test, and Wilccoxon signed ranks test.

The important results of this study are as follow:

¡¤There was no relation ship between the pain scores pre and post test the reflexology massage according to the General Characteristics of the patients - sex, age, education, job, marriage, religion, in come or diagnosis, operative name.

¡¤In the expeimental group there were significant decreases between the pre and post test for pain intensity (Z=3.941, P=.000) and there was difference between the two groups on the post test (U=17.0, P=.000).

¡¤The systolic blood pressure (Z=-2.300, P=.021), diastolic blood pressure (Z=-2.326, P=.020) and pulse rate (Z=-3.987, P=.000) also significantly different but there was no difference between the two Groups on the post test (U=157.0 P=.239). The quality of subjective pain measure by seven subclasses, sensory, affective, miscellaneous dimension, ordinal scale, number of pain terms and ratio scale decreased significantly compared to before the hand reflexology massage. The instrument used for pain quality in the experimental group there were significant decreases between the pre and post test and there was difference between the two groups on the post test (P=.001, P<.05).

¡¤The mood score was significantly improved after hand reflexology massage method (Z=-3.86, P=.000), and the difference between the two groups was also significant (U=68.0, p=.000).

In conclusion, there was a statistically significant difference in pain intensity, pain quality, blood pressure and pulse rate and mood. On the basis of the above findings, this study suggests that hand reflexology massage can be on effective method of relieving and improving mood in clinical practice in patient who have had spinal surgery.
